ALL kindergarten students, 7th Grade students, students enrolling from out-of-state, and all junior or senior high students participating in athletics must have a 2010-2011 physical form on file with the school.
In addition, ALL students must show proof of the varicella vaccination or of having had chicken pox. Another change for all students entering 7th grade is the required booster shot for diptheria, tetanus and acellular pertussis (DTaP) vaccination. These two changes are in addition to the previously required immunizations.

The documents for the Junior/Senior High School consist of two pages while the Elementary School document is one page. Please print, complete, sign and then return the required documents to the Milford Public Schools.
